3. No Emergency Services
The services provided are not intended for medical emergencies.
If you are experiencing a medical emergency, including but not limited to chest pain, difficulty breathing, severe bleeding, or signs of stroke, call 911 or go to the nearest emergency department immediately.
4. Eligibility for Services
You must be at least 18 years of age to independently use the services unless otherwise permitted by law.
By using the services, you represent that:
- You are legally able to consent to medical treatment
- You are physically located in a state where the provider is licensed at the time of the visit
- The information you provide is accurate and complete
Services for Minors
Services may be provided to patients under the age of 18 only with the participation and consent of a parent or legal guardian, as required by law.
The parent or legal guardian must be present and available during the visit and must have the legal authority to consent to medical treatment on behalf of the minor.
Certain services for minors may be limited or unavailable based on legal, clinical, or regulatory requirements.
5. Telehealth Consent
By participating in a virtual visit, you acknowledge and consent to the use of telehealth technology for the delivery of healthcare services.
Telehealth involves the electronic transmission of medical information for evaluation, diagnosis, and treatment. While safeguards are in place, there may be limitations or risks inherent in the use of electronic communications.
You may withdraw consent to telehealth at any time by discontinuing use of the services.
6. Medical Decision-Making

The provider may determine that telehealth is not appropriate for a particular condition and may recommend in-person evaluation or emergency care.
The absence of a physical examination may limit diagnostic certainty in some situations.

Coastline Virtual Urgent Care does not guarantee:
- a specific diagnosis
- a particular treatment plan
- the issuance of prescriptions
- resolution of symptoms

Medications may be prescribed when clinically appropriate and permitted by law.
Controlled substances are not prescribed through our services.
Prescription decisions are made solely at the clinical discretion of the treating provider and are not guaranteed.
